📝 Documents Required for NHM Kendrapara Recruitment

Candidates must attend the interview with the following documents:

  • Filled-up application form.
  • All original certificates & mark sheets for verification.
  • Self-attested xerox copy of all testimonials (certificates & mark sheets of all educational qualifications).
  • Two recent passport size colour photographs, duly self-attested.
  • Self-Attested photo ID proof (Voter ID card / PAN card / Driving License / Aadhar card/ Passport).
  • No Objection Certificate (NOC), if applicable (for candidates already working in the Health Department on regular or contractual basis).
  • A certificate for conversion to percentage of marks, if marks are in the form of CGPA, OGPA, etc..
