The candidates are required to send their application in prescribed format duly filled in with self- attested certificates, mark sheets & other required documents should be submitted to the Superintendent, Special Adoption Agency (SAA), Utkal Balashram Balasore Sakha, AT/PO- Sahadevkhunta, Dist-Balasore, PIN-756001 on or before 30/05/2025.
The application form must be submitted with the following documents:
- Aadhaar photocopy that has been self-attested
- self-attested copies of the mark sheets and certificates for secondary, high school, graduation, post-graduation, or other professional qualifications.
- A recent color self-attested photo must be attached to the application.
- Experience certificate from previous organization
- The employer’s certificate of no objection.
- Self-statement that one is not involved in any illegal activity, particularly offenses involving children.
- Any Gazetted Officer may issue a character certificate.
