1. For the positions listed at Serial Nos. 01 to 06, interested and qualified candidates must apply in the prescribed format and send a copy of their self-attested photocopies of the necessary testimonials to the CDM & PHO, Nabarangpur AT / PO-Mirganguda District:-Nabarangpur. PIN-764059 by 05.00 PM on or before August 18, 2025, via registered mail or speed post only. The application envelope must be superscribed with the name of the post for which the applicant is applying. Any sort of incomplete application will be denied.
2. On dt-05.08.2025, interested candidates for the positions listed in Serial Nos. 07 to 11 may attend the WALK-IN-INTERVIEW at the CDM& PHO Nabarangpur office chamber at the time and date specified against each. In order to be verified during the interview, candidates must present their original certificates, biodata in the format indicated (the same as the application form), and a set of self-attested photocopies of the necessary testimonials.
